Test Modification Instructions

For Students:

Two working days prior to the exam, turn in the completed Test Modification Form to Disability Services. Each test requires its own form. The steps to take are:

  • Talk with your Disability Services Coordinator about test modifications.
  • Get a Test Modification Form from the Disability Services office or download the form.
  • Identify yourself to your instructor as a person with a disability requesting test modifications.
  • Negotiate with your instructor how the test and test modifications will be administered.
  • Make certain all relevant items on the test modification form are complete and ready for Disability Services.

For Faculty:

Federal and State civil rights laws and University policy mandate that reasonable accommodations must be provided to otherwise qualified students with disabilities. Reasonable accommodations assure equal access to University courses and do not interfere with academic standards or freedoms.

For the convenience of instructors, Disability Services offers a test modification service. Instructors may either administer accommodations through Disability Services or independently.

Instructors may ask students for verification of the right to reasonable accommodations. Disability Services provides a letter of verification to students for this purpose. Exactly which accommodations will be provided are a matter of negotiation between instructors and students with Disability Services serving as a resource for both parties.

Complete the test modification form fully. Incomplete forms will be returned to the student for completion.

Disability Services adheres strictly to the instructor's testing directions. Changes to test modifications must be pre-approved by instructors even when the modification is as simple as a scheduling change.

Due to finite resources, Disability Services may not be able to administer an exam as per your directions. For instance, we may not have a room available for the test at the time specified by instructors. Should this happen, Disability Services will contact instructors and students to make alternative arrangements.
