Score Report Request Forms:
Score reports for up to four (4) graduate schools/programs are included in the basic TOEFL test registration fee, and may only be designated on the day of the test. Occasionally, a GRE examinee may want to send scores to an institution that is not listed on the Designated Institution (DD) List. In this case, the testing center will give you a Score Report Request Form to fill out. There is no fee as long as the examinee has selected no more than a total of four score recipients for the test fee. This form, after completing, must be returned to test administrator before leaving center.
Additional Score Reports:
Examinees who want to have scores sent to more than four recipients should complete the Score Report Request Form on page 61 of the TOEFL CBT Bulletin and return it to ETS together with the correct payment.
